The map below lists federally qualified health centers (FQHCs) and the services they provide related to substance use treatment and care, and HIV/STI screening, prevention, and treatment.

If you would like to update your FQHC’s listing or add a new listing, please click the “+Add” button above the map. You can create an account that will allow you to continue to make updates to your listing.
